>vices as needed. The Lead Pharmacist performs a wide range of pharmaceutical care functions to assur

>tion to patients and health providers, collaborating with clinicians on drugs of choice and therapeu 
>tic plans, ensuring access to medications, educating patients on the proper use of medications, and  
>ensuring that patients understand cautions, adverse reactions, and interactions related to their the 
>rapy. The Lead Pharmacist will be part of an interprofessional team consisting of physicians, advanc 
>ed practice providers, nurses, and other professionals. The Lead Pharmacist will be based primarily

5Required Qualifications : - Minimum 3-5 years of directly related experience. - Must have graduated 
>from an accredited school of pharmacy and possess a Doctor of Pharmacy degree (PharmD) or equivalent 
> degree as determined by the National Board of Pharmacy. - For graduates in a non-US country or juri 
>sdiction, pharmacists must have earned a Foreign Pharmacy Graduate Examination Committee (FPGEC) Cer 
>tification as determined by the National Board of Pharmacy. - Completed an ASHP Accredited Postgradu 
>ate Year 1 (PGY1) Pharmacy Residency Program or equivalent practice experience - Current certificati 
>on for Basic Life Support (BLS) from the American Heart Association. - Must possess a current licens 
>e in good standing to practice pharmacy in the State of California Preferred Qualifications : - Comp
