>psychosocial histories; conducts comprehensive mental status examinations; and formulates diagnoses  
>in accordance with the Diagnostic and Statistical Manual of Mental Disorders (DSM-5-TR). The incumbe 
>nt develops, implements, and monitors individualized treatment plans; provides individual, group, an 
>d family interventions, as appropriate; participates as a member of a multidisciplinary treatment te 
>am; collaborates with psychiatrists, nurses, peer specialists, and other behavioral health professio 
>nals to coordinate client care; serves as a liaison with inpatient psychiatric units, community-base 
>d organizations, and other service providers to facilitate referrals, care transitions, and continui 
>ty of treatment; provides ongoing case management, crisis intervention, and care coordination servic 
>es; advocates for clients in accessing behavioral health, medical, housing, vocational, and other su 
>pportive services; maintains accurate, timely, and confidential clinical documentation in accordance 
> with California state law, City and County policies, Medi-Cal documentation standards, and departme 
>ntal requirements; participates in quality improvement and utilization review activities, as assigne 
>d; attends required staff meetings, clinical supervision, and professional development trainings; ma 
>intains compliance with applicable professional licensure, certification, and continuing education r 
>equirements; and performs other related duties as assigned.

5MINIMUM REQUIREMENTS: - CSW I: Masters degree in social work, psychology, or counseling or related f
>ield from an accredited school of social work or equivalent experience/training and knowledge and ab 
>ilities essential to the successful performance of the duties assigned to the position - CSW II. A M 
>aster's degree in social work, psychology or counseling from an accredited school of social work and 
> possession of the requirements necessary for application for licensure as a clinical social worker, 
> as determined by the California Board of Behavioral Science Examiners; or an equivalent combination 
> of education and experience; and knowledge and abilities essential to the successful performance of 
> the duties assigned to the position - Ability to work cooperatively and communicate effectively wit 
>h others. Ability to work in a high stress, high volume area and work well under pressure and with d 
>eadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able 
> to efficiently prepare written summaries of activities, set priorities and complete reports within  
>required timeframes. Knowledge of medical and/or psychiatric terminology - Excellent communication a 
>nd interpersonal skills - Able to work as a team member in a culturally diverse setting - Able to li 
>ft 50lbs and climb stairs - Demonstrated excellent attendance and reliability - A preliminary job of 
>fer and assignment may be extended pending successful background clearance. - An understanding of me 
>ntal health issues and sensitivity toward mental health clients - Ability to work with clients and p 
>rogram staff from diverse cultural and social class backgrounds PREFERRED: - Two years working with  
>severely mentally ill adults, the homeless and dual diagnosed populations - Case Management and Care 
> Coordination experience - Culturally competent in working with clients from diverse cultural backgr 
>ounds and adverse lived experience. - One year experience working with African American clients - On 
>e year experience doing community-based case management and the ability to do outreach including to  
>SRO hotels - CSW II: Eligible to test for licensure with the Board of Behavioral Science as An Assoc 
>iate Clinical Social Worker (ACSW) or Intern Marriage Family Therapist (IMFT). LICENSES/CERTIFICATIO 
>NS: - CSW II. A Master's degree in social work, psychology or counseling from an accredited school o
